Social Work in the Health Care setting addresses the physical, mental, emotional, and social well-being of the patient/residents, families or caregivers.
A registered Social Worker offers the following services at the hospital:
- Psychosocial Assessment
- Crisis Intervention
- Assessment and Referral to community agencies
- Multidisciplinary Discharge Planning
- Case Management
- Patient Advocacy
- Practical Assistance (eg. Advance Care Directives, Housing, financial issues)
- Stress Management
- Education/Health prevention and promotion
- Counseling
- in patient ( eg. Grief/bereavement, depression/anxiety, eating disorder, adjustment etc.)
- outpatient, short term duration
- critical incident stress debriefing/diffusing
Other responsibilities include coordinating the volunteer programs:
- Hospice Northwest Palliative Care
- Volunteer (recreation and social activities)
