Information

 

 

What qualities are we looking for?

Patients, families of patients or care partners that have experienced care within the past two (2) years who are:

 

  • Committed, reliable and trustworthy and believe that their investment will help in making a difference for other patients and families.
  • Willing to share insights and information about care experiences in ways others can learn from.
  • Able to see beyond their own personal experiences and show concern for more than one issue or agenda.
  • Good listeners and respect perspectives of others.
  • Comfortable speaking in group and interact well with others.
  • Able to work in partnerships with others.
  • Positive and supportive of a model of care that is patient and family centred.
  • Able to maintain confidentiality of patient and organizational information.

 

Time commitment:

  • May be a one-time activity, such as sharing your hospital experience to healthcare providers or a council/committee as a patient or family consultant.
  • Once becoming a Patient Family Advisor (PFA), attend Patient Family Advisor meetings.
  • Involvement suited to individual’s availability.
  • May involve participation in a monthly meeting.
  • May be involved in a number of committees, working groups or teams.

 

Application Form/Reference:

  • Interview.
  • Complete the application form.
  • Submit two (2) completed/signed reference forms (family members are not acceptable).

 

Orientation and Training Sessions:

  • Geraldton District Hospital Mission, Vision and Values.
  • Overview of Patient and Family Centred Care (PFCC).
  • Role of Patient and Family Advisors.
  • Patient Family Advisory Council.
  • Confidentiality and sign form.
  • Orientation with volunteer services.
  • Specific training related to area of interest/participation.

 

How to Become an Effective Patient Family Advisor:

  • Become a partner to improve the experience of care for all.
  • Share your experiences in ways others can learn.
  • Develop good communication and listening skills.
  • Be open-minded and have a positive approach.
  • Seek to find solutions and provide change.
  • Support Geraldton District Hospital in providing excellence in patient care.
  • Be an ambassador and example to others to become Patient Family Advisors.
  • Stay connected, attend and participate in quarterly Patient Family Advisor Council meetings.
  • Notify the PFCC Lead of any changes in your situation (i.e.: availability, contact information, areas of interest/participation, etc.).

 

Patient and Family Advisors Role:

Patient and Family Advisors will work in partnership with healthcare providers. This collaborative role will result in a more efficient planning and assist to meet the needs and priorities of our patients and families, community and region.

 

Patient and Family Advisors will have direct input and influence on the policies, programs and practices that affect the care and services that individuals and families receive at Geraldton District Hospital.

 

Patients or families of patients will have experienced care within the past two (2) years to participate in this role.

 

Patient and Family Advisors may assist in any or all of the following to enhance in the delivery of Patient Centred Care:

 

  • Bringing unique perspectives and expertise to the organization by sharing their hospital experiences and telling their stories.

 

  • Actively participating with healthcare providers on councils, committees, teams or focus groups.
  •  
  • Provide input into patient care, organizational processes, education, research, patient safety, quality improvement, staff orientation and recruitment.
  •  
  • Help to improve the environment and design of the organization.
  •  
  • Participate in a mentoring or supportive role with other Patient Family Advisors.

 

 



Patient Family Advisor Council

 

Terms of Reference

 

Purpose:

  • Patient family advisors (PFA) will represent the voice of the patients and families of Geraldton District Hospital. They will be involved in the implementation and continuing advancement of Patient and Family-Centred Care (PFCC) in the hospital through support of our Mission, Vision and Values.Principles:
  •  
  • Respect and dignity, information sharing, meaningful participation and collaboration.
  • Better health: improve population health through health promotion, protection and disease prevention, collaborating with communities and different government organizations.
  • Better care: in partnership with patients and families, improve individual’s experience, achieve timely access and continuously improve healthcare safety.
  • Better teams: build safe, supportive and quality workplaces and develop a highly-skilled, professional and diverse workforce that has a sufficient number and mix of service providers.
  • Better value: achieve best value for money, improve transparency and accountability and strategically invest in facility, equipment and information infrastructure.
  • Be respectful of one another.
  • Focus on processes, not people.
  • Be open-minded to new ideas.
  • Be results-focused and action-oriented.
  • Be a community.
  • Be honest in dialogue.
  • Do not focus on personal agendas.
  • Come to meetings prepared, on time and informed.
  • Maintain commitment to the Council.
  • Allow everyone the opportunity to speak and ask questions.
  • Follow the agreed upon process for making decisions.

 

Responsibilities:

  • Make recommendations to the PFCC Council in regards to organizational priorities, including improvement strategies through the Quality Improvement Plan (QIP).
  • Advise on issues referred by senior management or the PFCC Council. PFA consultation will be coordinated through the PFCC Lead.
  • Share ideas for the improvement of services with a patient and family focus in mind.
  • Help review and develop policies and procedures that integrate the PFCC culture.
  • Provide an annual evaluation of the PFA’s functioning and develop an action plan to remedy potential shortcomings.
  • Prepare presentations and report to the Board of Directors, as requested. This will include an overview of measures, policies and procedures and hospital practices at Geraldton District Hospital that incorporate with PFCC.
  • Continue to evolve and implement best practice PFCC strategies.
  • Declare any conflict or potential conflict of interest.
  • Decision-Making:
  • Decision-making will occur through discussion and consensus whenever possible.
  • Discussions will be supported through measurement data and patient, family and staff satisfaction.
  • In the event that consensus cannot be reached, the Chair will introduce additional processes and structure to reach a decision.
  • All requests for PFA involvement and participation will be coordinated through the PFCC Lead.
  • Sub-committees may be formed and operated to deal with specific issues, as required, and will be created and accountable to the PFA Council.

 

Membership:

The Patient Family Advisor Council will have representation from:

 

  • Community members (1-3); one of which is an Elder from a reserve in Greenstone.
  • PFA (Co-Chair)
  • PFCC Lead (Co-Chair)
  • Senior Management

 

A Patient Family Advisor will co-chair the Council with the PFCC Lead. PFA Council members who are appointed to their position will serve an indefinite term. A member may be removed through failure to abide by their duties and responsibilities as PFAs. Representative members will rotate on a two (2)-year term with an option of reappointment.

 

PFA Co-Chair Responsibilities:

  • Work in collaboration with the PFCC Lead in co-chairing PFA Council meetings.
  • Provide updates and share information with the PFCC Council.
  • Represent GDH and the PFA Council members in official endeavors.
  • Assist in recruitment, orientation and mentoring of other patient family advisors.

 

Advisory Council Members Responsibilities:

All PFA Council members are expected to:

 

  • Meet all requirements to become a PFA (eligibility, orientation, education).
  • Maintain confidentiality and work within the policies and procedures of GDH.
  • Attend meetings of the Council and actively participate in discussions.
  • Participate in a minimum of 50% of the PFA Council meetings.
  • Continue to educate themselves on PFCC best practices and do research, as required.
  • Share their perspective of their hospital experiences.
  • Mentor and support other PFAs.
  • Apply innovative thinking.
  • Serve as an ambassador for patient and family-centred care.
  • Sign letter of confidentiality.
  • Obtain a clear criminal reference check prior to first meeting.

 

 

Membership Selection Process:

  • GDH will seek interested individuals among patients and families.
  • Nominations and recommendations from staff and PFCC Council members will be taken.
  • Postings through the media and other mechanisms will be used to advertise for potential membership.
  • Interested individuals will be asked to complete an application and will be invited to a session, where information will be shared and the potential involvement of the individual will be discussed.
  • Once the PFA Council is in operation, selection of new members will be jointly carried out by the PFCC Lead and the PFA co-chair.Meetings:

  • The PFA Council will meet a minimum of six (6) times per year and as requested by the PFA Council members.
  • Meeting agendas and minutes of the meetings will be circulated to Council members.
  • Clerical support will be provided.

 

 



 

 

Tips for How to be an

Effective Patient Family Advisor

 

A Beginning Checklist

 

 

  • Believe that your investment in your own health experience or your family members’ will help you in making a difference for others.
  • Develop good communication skills.
  • Listen with empathy.
  • Be honest.
  • Be available.
  • Ask questions.
  • Be open-minded.
  • Avoid assumptions. Do not judge someone based on appearance.
  • Learn to be comfortable with staff and other patients and families.
  • Be willing to partner with other patients and family members.
  • Learn negotiation skills. Be a team player.
  • Think about the points you want to make.
  • Adapt to different situations.
  • Take responsibility for learning.
  • Give input based on your own experience but be able to step out of your individual perspective. Do not deal with personal issues in advisory activities.
  • Tell your own story or be clear if it is someone else’s experience.
  • When talking about an experience that did not go well, do not complain. Offer suggestions on how it could have been made better.
  • Think carefully about the words you use. Anger is not productive. If you want to tell a story that brings up strong emotions, ask a peer to help develop your story.
  • When speaking with a group, thank them for asking for your input.
  • Support staff.
  • Be willing to partner with staff.
  • Honor commitments; when you say you are going to do something, follow through.
  • Have faith. Expect the best.
  • Stick to it.
  • Ask for help or support from other advisors when you need it. Do the same for others.
  • If you find that you are having difficulty balancing your personal and family life with advisory activities, let the PFCC Lead know, take a break or give up some PFA responsibilities.

Volunteer Positions Available

The Geraldton District Hospital is accepting applications for up to five (5) Patient Family Advisors.

This volunteer position works collaboratively with the healthcare team to influence and impact the care and services that clients receive at the Geraldton District Hospital.

 

Criteria:

 

  • Patients and families of patients that have experienced care within the last two (2) years
  • Positive public relations skills
  • Ability to be comfortable participating and engaging in collaborative environments and meetings through discussion and sharing experiences

 

The successful candidates will participate in a team environment within the Volunteer Services department.
